Our office is committed to helping you maximize your insurance benefits.

Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Our competent office staff will handle as many of your insurance questions and needs as possible.

We do participate with most health insurance plans. Please contact your insurance company to find out if our physicians participate with your plan. In order for us to determine benefits, please have your insurance card available when calling to schedule your appointment. This will help us verify your benefits before your visit and reduce your wait time during the check-in process. Although we have verified your insurance, you must still bring your insurance card and driver’s license with you. These documents will be used to verify your identity. We do this to protect you and your family from insurance and identity fraud.

Co-pays, co-insurances and deductibles are due at the time services are rendered. For our patients who have an 80/20 plan or with a high deductible, we request payment in full at the time of service until the deductible is met. Payments can be made with cash, check, Visa, and Mastercard.

You will be considered responsible for all visits and procedures not covered by your insurance. It is in your best interest to fully read and understand all aspects of your health insurance coverage.
